Dear Parent/Caregiver/Referrer On this page you'll find forms to fill out and sign to help me gather information relating to you and the person or young person who brought us together. You need to complete the Intake and Payment Forms prior to our first appointment. I will tell you how to fill the forms out during our first telephone or email conversation. The confidential information you share will allow me to review the referral situation in advance, as well as lead our discussions during our 1st meeting.
